Under-Eye Filler in Istanbul
The under-eye area can look hollow, shadowed or tired for many different reasons. Tear trough anatomy, cheek support, skin quality, pigmentation and fluid retention must be assessed separately before filler is considered.
At our Akasya Acibadem clinic in Uskudar, Istanbul, the objective is subtle correction in carefully selected patients rather than simply adding volume beneath every dark circle.
What Is Under-Eye Filler?
Under-eye filler usually refers to placement of a suitable hyaluronic acid product in the tear trough or adjacent support areas. It may soften a structural hollow but cannot remove pigment, prominent eye bags, loose skin or every type of shadow.
The periorbital area has important blood vessels and a tendency to retain fluid. Conservative product choice, depth and volume are essential.
Who Is It Suitable For?
Suitability is confirmed through an individual medical assessment. Treatment may be considered for:
- Selected patients with a true structural hollow and appropriate skin quality
- People with stable expectations for a subtle rather than complete correction
- Patients without active infection, marked oedema or an untreated eye-area condition
Medical Assessment and Treatment Planning
Assessment is performed in natural light and includes the cheeks, eyelid–cheek junction, lower-eyelid bags, skin thickness and facial expression. Previous filler and any history of persistent swelling are important.
In some faces, cheek support, skin-quality treatment or no injection may be safer and more appropriate.
What Happens During Treatment?
After cleansing, small amounts of product are placed with a needle or cannula according to anatomy. The procedure is deliberately conservative and additional correction, if appropriate, is considered only after swelling settles.
Expected Results and Timeline
An early change may be visible immediately, but swelling can distort the first impression. Final assessment is made after the area has settled. Hyaluronic acid filler is temporary and longevity varies.
Downtime and Recovery
Bruising, tenderness and swelling are common early effects. Under-eye puffiness can last longer in susceptible patients, so treatment should not be scheduled immediately before an important event or flight.
Possible Side Effects and Limitations
No medical aesthetic treatment is risk-free. Possible effects and limitations include:
- Bruising, swelling, tenderness or asymmetry
- Visible product, lumps or a blue-grey Tyndall effect
- Persistent oedema or contour irregularity
- Infection or delayed inflammatory reaction
- Rare vascular occlusion with skin injury, visual impairment, blindness or stroke
Aftercare
Do not press or massage the area unless specifically instructed. Seek urgent assessment for increasing pain, blanching or mottled skin, visual change, severe headache or any rapidly worsening symptom.
Frequently Asked Questions
Does filler treat dark circles?
Only when a structural hollow is a significant part of the shadow. Pigment and vascular colour need different strategies.
Can it remove eye bags?
No. Prominent fat pads or loose skin may make filler unsuitable and may require another approach.
Can under-eye filler be dissolved?
Many hyaluronic acid fillers can be treated with hyaluronidase, but reversal has its own limitations and risks.
Dark circles, puffiness and hollows require different treatment
A dark under-eye appearance may be caused by pigmentation, visible blood vessels, shadows from a tear trough hollow, skin laxity or fluid retention. Filler adds volume; it does not erase melanin, treat all vascular discolouration or remove under-eye fat bags. The first step is identifying which issue is dominant. In people with marked puffiness, additional filler can sometimes make the appearance worse.
Why the under-eye area needs particular caution
The periorbital region has delicate tissues and significant vascular connections. Injectables can cause prolonged swelling, blue-grey discoloration, nodules, asymmetry and rarely vascular injury affecting vision. The US FDA has not approved fillers for injection into the periorbital area; the regulatory status of individual products in Türkiye needs independent verification. Serious or unusual pain, skin blanching or a vision change after any facial injection requires emergency medical assessment.
Alternatives to tear trough filler
When the dominant concern is superficial skin texture, the discussion may involve conservative skin care or carefully assessed under-eye mesotherapy; these do not automatically solve structural hollowing. Significant fat bags or excess eyelid skin may warrant oculoplastic or plastic surgical assessment. The aim is to avoid using extra volume as a universal answer to a tired appearance.
